I hereby authorize the National Naval Aviation Museum, the Naval Aviation Museum Foundation, and all affiliates, contractors, or third parties of the Glenn Miller Orchestra to publish photographs and videos taken for use in print, online, and video-based marketing.

I further acknowledge that my participation is voluntary and that I nor my registered guests will receive financial compensation of any type associated with the taking or publication of these photographs.

Important information

FOR THOSE WHO DO NOT HAVE BASE ACCESS Ex. DOD ID or CAC Card:

Everyone attending the concert who does not have Department of Defense ID card (DOD ID) will have to fill out a 5512.

Base Security requires you to fill out a 5512 form if you do not have a Department of Defense ID card (DOD ID). The required form is attached to the bottom of the home page, and should be included in your confirmation email.

If you show up to the gate for the Glenn Miller concert without filling out a 5512 form; the gate guards will direct you to the VCC at the Main Gate to fill out the required forms.
